Position Overview:


We seek a compassionate and detail-oriented Nurse, who will be tasked with assessing, planning, implementing, and evaluating patient care plans in collaboration with a multidisciplinary healthcare team.


Key Responsibilities:

Patient Assessment and Care


Effective use of the admission and discharge protocol on the EMR
Monitor patients' conditions and assess their needs, provide optimal care and advice.
Observe and interpret symptoms as well as communicating them to physician.
Creating individual care plan for patients in collaboration with other team members e.g. medical officers, occupational therapist, physiotherapist, speech therapist, dietitian, etc.
Accompany In-patient's on follow up appointment.


Clinical Procedures


Perform routine baseline vital signs check e.g. Temperature, Pulse, Respiration, Blood pressure, SPO2, Fasting blood sugar (FBS) on resumption and also at the close of shift.


Wound care.


Medication administration as prescribed by physician including; oral, intravenous, intramuscular, suppository, etc.
Meal; Percutaneous Endoscopic Gastrostomy (PEG) and assisted feed for patient with dysphagia.


Tracheostomy care.


Resuscitation; suctioning, administering oxygen and nebulization as prescribed by physician.
Cannulation: Phlebotomy, monitoring intravenous fluid. Supporting medical officers to collect blood, and other specimen from patient.


Monitoring Intake and Output chart.


Routine Nurses ward round, Medical Officer, and or the Clinical Director as applicable.
Handover and taking over of patients at bed side.


Report writing

Safety and Hygiene


Maintain excellent hygiene and safety (decontaminating equipment, sanitizing surfaces, autoclave dressing packs and drums for sterile/aseptic procedures.
Case Incidence; patient's falls, medication errors, and or any other adverse events.
"And other duties as assigned by my supervisor, doctor, and or medical consultants".


Professional Development


Attend educational workshops, trainings (internal and external) and conferences to expand knowledge and capabilities.


Qualifications:


Bachelors of Nursing in Nursing services or a related field
Minimum of two (2) years of experience in Nursing services.
Self-motivated, goal-oriented and able to work independently
