Purpose of the position:
Responsible for implementing and monitoring the Case Management Model (CMM) through adherence support for ART patients until they are eligible for decanting to any of the differentiated models of care.
Minimum Requirements
Grade 12 certificate
Enrolled nurse qualification will be an added advantage
At least 2 years' relevant experience in counselling and testing
Adherence counselling
Basic HIV testing and counselling
Index case trailing
Able to communicate in English and isiXhosa is an advantage

Duties and Responsibilities
Capture details of clients on ART for less than one year and newly initiated patients on ART on the CMM excel spreadsheet, and plot all due appointments
Capture clients with high viral loads, pediatric clients, pre-ART clients and pregnant women
Phone clients and conducts daily linkage to a CHW or any appropriate community person for those who missed their appointment; pay a visit to clients who missed their appointments, patients who are not virally suppressed, and patients needing additional care
Issue routine reminders according to the schedule of appointments by sending SMS reminders
Responding to messages from clients and crafting an appropriate and professional response
Refer patients for enrolment in support groups according to the patient's preference
