Responsibilities:
Assess Care needs
Assist families to implement recommended care to ensure safe discharges are in place.
Connect patients to relevant resources such as care agencies, frail cares, day clinics, palliative care resources etc.
Make referrals to appropriate service providers such as psychologists, Department of Social Development etc.
Provide short term counseling
Provide mediation to patients and families
Provide practical assistance to patients to reduce stressors in order to participate in their rehabilitation.
What We're Looking For
SACSSP Registration (current and valid)
Professional Indemnity Insurance
Training and experience in Palliative Care (essential)
Additional experience in Oncology, Curatorship, or Geriatric/Frail Care (advantageous)
Proven ability in Hospital Social Work and Case Management
Strong report writing and time management skills
Comfortable working flexible hours within normal business times
Willingness to travel for outpatient consultations
Ability to collaborate effectively with an interdisciplinary care team
