Responsibilities
Conduct psychosocial assessments and contribute to discharge planning.
Provide counselling, emotional support and mediation for patients and their families.
Coordinate multidisciplinary care and facilitate referrals to appropriate community and healthcare services.
Identify and address barriers to recovery, treatment adherence and successful discharge.
Maintain accurate clinical documentation and participate in multidisciplinary team discussions.
Advocate for patients' social, emotional and practical needs throughout their care journey.

Requirements
Current SACSSP registration.
Professional indemnity insurance.
Minimum of 3 years' social work experience.
Experience in hospital social work and case management.
Training and experience in palliative care (essential).
Strong report writing and time management skills.
Ability to work collaboratively within a multidisciplinary team.
Willingness to travel for outpatient consultations.
Experience in geriatrics, oncology and curatorship will be advantageous.
Compensation
60% commission on services rendered.
